The birth place named as Оmiłów is unknown; the place of death named as Załuż is actually Załuże, a suburb of Rohatyn known in Ukrainian as Залужжя (Zaluzhzhia).
AGAD record 300.0.4300 page 3 lists the death of Hersch Reichbach, son of Mechel, on 13 May 1936, in Załuże-Sołońce.
