Other Notes:
[N3] 'God' is inscribed 'ד rather than 'ה, the standard abbreviation for Hashem השם [The Name]. Appearing only in writing, 'ד represents an even-higher level of 'distancing' than that provided by 'ה, but is nevertheless voiced 'Hashem'.
[N4] The 'ב of this abbreviation probably stands for 'son of'. The ה'מ' is an honorific.
[N6] The 'Counting of the Omer' commences on the second evening of Pesach [the second Seder in the Diaspora], each evening for seven weeks, the following day being Shavuot. The 33rd day is a 'minor festival' celebrated for the historic events that are traditionally-held to have occurred on this day. It is known as ל'ג בעומר, Lag Ba'Omer – Lag being the Hebrew pronunciation of the number 33. Usually, a person who died on this day will have this alternative Lag Ba'Omer inscribed on his/her gravestone, rather than the equivalent Hebrew calendar date which is the 18th of Iyyar.
[N6] The year 5698 appears as תרח'ץ [600+8+90] rather than the usual hundreds/tens/units format תרצ'ח [600+90+8], because the latter spells the Hebrew word for 'murder' or 'kill'. תרצ'ח was therefore often avoided on gravestones that year, throughout the Jewish world, for fear of 'the evil eye' עין הרע.
